Common Questions Male Hair Transplant Patients Ask Dr. Zandifar
In this ZMDHair video, double board-certified facial plastic surgeon Dr. Hootan Zandifar addresses the questions he hears most often from men considering hair restoration. Trained at UCLA and a member of the American Society of Hair Restoration Surgery, Dr. Zandifar brings specialized expertise to every consultation at his Beverly Hills practice.
Whether you are curious about how natural the outcome will look, whether a scar will be visible, or how permanent your new hair will be, Dr. Zandifar’s answers—drawn directly from his clinical experience—provide a clear, honest foundation for your decision.
- Modern FUE hair transplants produce natural-looking results that are undetectable to others.
- FUE harvests follicles individually, avoiding a linear scar and allowing shorter hairstyles post-procedure.
- Transplanted grafts come from testosterone-resistant donor areas, making results long-lasting when hair loss is hormone-related.
- Combining a transplant with hair-loss prevention strategies is essential to protect results over a lifetime.
Will a Hair Transplant Look Natural?
As Dr. Zandifar states in the video, modern hair transplantation is designed so that “no one will know you had any hair transplantation done.” The FUE (follicular unit extraction) technique plays a large role in achieving this outcome because individual follicular units are harvested and placed to mimic the natural angle, direction, and density of existing hair.
Artistic hairline design is equally important. A skilled surgeon considers the patient’s facial proportions, age, and long-term hair-loss pattern before placing a single graft, ensuring the result integrates seamlessly with native hair rather than appearing pluggy or uniform.
FUE vs. Strip: Understanding the Scar Difference
Dr. Zandifar explains that with FUE, follicles are harvested individually, leaving no linear scar—though he notes that if the head is shaved completely, small circular marks may be visible on close inspection. The strip (FUT) method, by contrast, removes a linear section of scalp, leaving a scar that limits how short a patient can comfortably cut his hair.
- FUE: Minimally invasive, no linear scar, suitable for patients who prefer very short hairstyles.
- FUT/Strip: May allow harvesting of a larger number of grafts in a single session but results in a linear donor scar.
- Consultation matters: The right technique depends on donor density, the extent of hair loss, and personal lifestyle preferences.
How Long Do Hair Transplant Results Last?
Dr. Zandifar explains that transplanted hairs are taken from donor areas genetically resistant to the effects of testosterone—the same hormone responsible for androgenetic (male-pattern) hair loss. Because of this resistance, he states the new hairs “will stay with you for the rest of your life” as long as the underlying cause of loss is testosterone-related.
Critically, he also cautions that existing non-transplanted hair can continue to thin over time. For this reason, he recommends combining every hair transplant with strategies to prevent further hair loss, protecting the overall result long-term. Patients should discuss evidence-based medical management options with their surgeon at the time of consultation.

the most common questions my male hair transplant patients are asking me are that first of all is it natural and the answer to that question is absolutely hair transplantation these days is natural no one will know you at any hair transplantation done second of all is there any scar well if you utilize the follicular unit extraction or the Fue technique where the hair follicles are individually harvested there's really no visible scar now if you shave your head completely maybe you can see the little spots but typically you cannot if you utilize the strip technique where a big strip of hair is taken then there will be a scar and so you won't be able to shave your head completely to uh avoid uh knowing that you had a hair transplantation um another question that's common and I'm asked is how long do the results last well the good news is that the new hairs that we transplant are from areas that are resistant to the effect of testosterone so as long as your hair loss is due to the effect of testosterone the new hairs will stay with you for the rest of your life now you may lose the hair that you have up there already and so every hair transplantation should be combined with techniques to prevent further hair loss to make your results last a lifetime

Frequently Asked Questions
- Will people be able to tell I had a hair transplant?
- According to Dr. Zandifar, contemporary hair transplantation is natural enough that no one will know the procedure was performed. Proper graft placement, hairline design, and technique selection all contribute to an undetectable outcome.
- Does FUE leave a visible scar?
- Dr. Zandifar explains that FUE leaves no visible linear scar. He notes that very small circular marks may be detectable if the head is shaved completely, but they are not typically noticeable with normal hair length.
- Are hair transplant results permanent?
- Transplanted hairs are harvested from donor areas resistant to testosterone, so Dr. Zandifar states they should remain for the patient's lifetime when hair loss is testosterone-driven. However, native hair that was not transplanted can still be lost over time.
- Why is hair-loss prevention important after a transplant?
- Even after a successful transplant, existing hair can continue to thin due to ongoing androgenetic hair loss. Dr. Zandifar recommends pairing every transplant with hair-loss prevention measures to maintain a full, natural appearance long-term.
